In endodontics, the term “danger zone” refers to the thinnest area of dentin thickness on the root of a tooth. You need to be aware of these danger zones because If you are not careful instrumenting this area, it can lead possibly lead to a strip perforation! For maxillary molars due to the irregular shape of the MB root, the danger zone is usually associated with the MB2. In one micro CT study, it showed that the dentin thickness on the distal aspect of the MB2 canal (danger zone) varied from 0.57 to 1.77 mm (median 1 mm) and from 0.61 to 1.77 mm (median 0.99 mm). The study also showed that dentin thickness of the MB2 canal was significantly thinner than the MB1 canal on both aspects of the root. (Ordinola-Zapata et al. IEJ 2018). The endo tips for today are: 1. Be aware that these danger zones exist! 2. Be careful to avoid excessive dentin removal with overflaring! Since this area is already thinner, overflaring with larger Gates Glidden or larger rotary instruments can weaken this area of the tooth or even worse cause a strip perforation. As you can see in the image posted, using rotary files with larger tapers and size can lead to minimal dentin thickness remaining. 3.
