Recently, I was helping a dental student who was having a hard time trying to buildup this lower premolar show in the images after all the decay was removed and the pulp was exposed. For dental students, in cases like this where there is no chamber left to hold in the irrigants and also if the root canal treatment will require more than one visit, I would recommend building up the tooth before starting your endo treatment to make it easier. The typical way students would try to buildup the tooth is place a cotton pellet or sponge in the pulp chamber space, and then place a matrix and fill the entire tooth with their buildup material. This results in more work trying to reaccess the tooth and also in narrow teeth, the cotton pellet or sponge can be trapped inside the buildup material and be hard to fully remove. What I like to teach students to do in cases like this with a single canal which is much faster and efficient: 1. Insert a rotary file into the coronal portion of the canal as far as it will go 2. Place your matrix band around the tooth 3. Use glass ionomer material to complete your buildup 4.
