Leeches used for localized venous congestion after surgery!! This video is of a hand where several fingers were cut off and then replanted/sewn back on. On the event of a venous failure after this procedure, the blood in the finger does not return to the body and is stowed. That can make the finger tense, bluish, swollen, occasionally blistering and the capillary filling is often rapid. This is where the leeches help to optimize circulation, by draining excess blood that accumulates in the finger. We insert a needle distally into the finger which will give the leech an attachment point. In addition to sucking out blood, the leech has a blood-thinning enzyme in its saliva that will help keep the finger from bleeding - which is good! Before the finger stops bleeding, a new leech must be put to work. The treatment with the leeches is maintained until the surgeons assess that the finger (or fingers if there are several) will manage on their own without external drainage.
